  • Biographical Statement (1500 words maximum)
  • Financial Statement (500 words maximum)
  • Tax return (pdf)
  • Most recent school unofficial transcript 
  • Three (3) reference emails (they will be emailed by submitting this form and will submit their letters of recommendation to the board).

Note: In order to be considered for the JSA Scholarship, you must be a student at Middle Tennessee State University. Emails must end with “mtsu.edu”.

Letters of Recommendation

Three letters of recommendation are required:

  • One attesting to your personal character,
  • One concerning professional employment or volunteer service, and
  • One describing your academic ability from an MTSU faculty member.

Letters from family members are not acceptable.
